Qualification for rehabilitation: step by step

The qualification process begins with contacting our Customer Service Office in Otwock or Kraków. Our consultants are available to guide you through every step and answer your questions:

Analysis of patient records

Qualification for an individually tailored programme is based on a detailed review of the patient’s medical records, an assessment of the current state of health, and (if possible) an interview with the patient and their family. In addition, our interdisciplinary team also uses functional-status questionnaires to help design a programme precisely matched to the patient’s abilities and needs.

Development of an individual therapy plan

If the qualification is positive, a team of specialists prepares a personalised rehabilitation plan that reflects the patient’s goals and clinical priorities.

Once the proposed plan is accepted, we begin the admission process. On the first day, all relevant specialists meet the patient, agree a shared therapeutic goal, and confirm any initial adjustments to the therapy plan.

We understand your situation. This is how we assess your potential.
We are here to help you realize it together.
